Does that mean it gives +1 to the req of the weapon u are holding? Life if your holding a sword your swordsmanship will increase one?

No, it gives +1 to whatever rune is attached to the headgear.

If you get a staff wrapping of mastery, or whatever the generic "+1 to attribute" mod is, then it looks to the required attribute of the staff it's applied to.

As far as hero headgear goes, I haven't tested it thoroughly, but I believe that if you do not put a rune into the headgear, that it then looks to the item the character is wielding.
